  • This form is to be used if you are making mass course changes. Contact the UCC chair before submitting this form to make sure you are using the right form. Select all of the changes below that apply and attach a memo that precisely describes your changes.

  • For the title, include a list of all courses that are being changed and the type of change (e.g., prereq or deletion).  As an example, the title might be “CSC 260, 360, 362, 364 prereq changes” or “CIT 470, 472, 480, 481, 485 deletions”.

  • User Tracking
